For children entering our in-school mentoring programs at partner schools in Greater Victoria, Sooke, the Comox Valley or Salt Spring Island, we need:

  1. their teacher or someone from their school to complete a referral form, and
  2. their parent/guardian to complete their Informed Consent, Media Consent and COVID-19 Waiver Forms.
