Developmental relationships survey

1. Express Care
How much conscious attention do you give to “express care” to your mentee overall? ( ie.show your mentee they can trust you; make them feel known and valued; give them focused individual attention)
2. Challenge Growth
How much conscious attention do you give to “challenge growth” when you are with your mentee? (ie. set high expectations for your mentee; help them learn from mistakes and failures; hold them accountable for their actions)
3. Provide Support
How much conscious attention do you give to “provide support” when you are with your mentee? (ie. guide mentee thru hard situations; build mentee’s self-confidence; advocate for your mentee)
4. Share Power
How much conscious attention do you give to “share power” with your mentee? (ie. take mentee’s ideas and opinions seriously; involve mentee in important decisions; let mentee take the lead)
5. Expand Possibilities
How much conscious attention do you give to “expand possibilities” when you are with your mentee? (ie. inspire your mentee to see future possibilities; expose your mentee to new idea’s and places; introduce your mentee to people who help them grow)

Six Conditions

Please check-in with the mentee about each of the Six Conditions but case-note only what’s not working, not meeting expectations, needs follow-up or is exceptional!
What activities have you been doing with your mentee? How are you engaging your mentee in the planning of activities? Do you need any help in activity planning?
1A. Activity Expectations
How often do you meet with your mentee? If you had missed visits, why? What keeps you motivated to see your mentee? Do you think you are getting enough support from BBBS and the school?
2A. Consistency Expectations
How would you describe your connection/relationship with your mentee? What are you doing to build a close relationship? Do you think they would talk to you if something was bothering them? Have they ever told you or have you noticed anything concerning? If so how did you handle it?
3A. Connectedness and Closeness Expectations
What new ideas, perspectives or activities have you proposed or engaged in that support your mentee’s development and growth? What do you tell your mentee they are good at? How do you celebrate your mentee’s uniqueness and talents? What have you learned from your mentee?
4A. Youth Centeredness Expectations
Does your mentee test boundaries or are you having trouble with their behaviour? Do you have any child safety concerns?
5A. Structure Expectations
Are your expectations being met? How or how not? What do you find fulfilling about this experience? How do you feel about the level of communication you are receiving from the agency? Are you wanting to continue with the program?
6A. Duration Expectations

ISM Specific Questions

If the mentee answers YES to any of the questions below, please describe their response in box labelled “Additional information from mentee”. These questions provide an opportunity to discuss boundaries and programs rules, if necessary, as well as any changes that the mentee wants in their match.
1. Have you had contact with your mentee outside of school (in-person, by email, over social media, gaming platforms, etc.)
2. Is there any activity that you want to do with your mentee before the end of the school year? (If applicable remind them to prepare for closure/wrap up)
3. Is there anything you want to talk about or do you have any concerns?
