Volunteer Transition Re-Assessment Interview

This form is to be completed by Volunteers, or by Staff on behalf of Volunteers, who wish to continue mentoring in a BBBS Site-Based Program (i.e. ISM or B You) with a new match in the next school year. Attempts should be make to complete interview via phone call, Teams/Zoom call, or in person.

(e.g. relationships, marital status, living arrangements, employment, family, etc.)
Probe: How will continuing with this commitment fit into your current schedule? Is there anything that might keep you from making and regular commitment moving forward?
Probe: Age ranges, characteristics, interests etc.
